What Information Do We Collect and How Do We Collect It?

This website has been created to collect data used to screen individuals for exposure to and symptoms of Covid-19. In order to collect this data, we ask individuals to register on the site and provide such information as their name, email address, type of user of the system (student, parent or legally responsible adult, staff member or visitor) and optional information such as the class, student ID or cohort/group of the student. We also ask for a language preference from the site’s available languages. And we ask for answers to health questions each day the system is utilized. All of this information is collected through our interface by providing responses into the appropriate fields.

Because of the way that World Wide Web communication standards work, when you arrive at or leave the Website, we automatically receive the Web addresses of the site that you came from or are going to. We also may collect information on which pages of the Website you visit while on the Website, the type of browser and device you use, your IP address, and the number of times you access the Website. The information referred to in this paragraph is called “Website Traffic Information.”

How Do We Use the Information?

We use the information collected to help determine, based on parameters for admission determined by the school, whether or not the respondent (or the child they are responding for in the case of a parent or legally responsible person) is cleared to come to the school where they may be subject to further screening. School staff reviews individual data per respondent and consolidations of the data to spot health trends, monitor overall compliance with government rules and regulations regarding screening and for contact tracing purposes.

TemPositions does not share personally identifiable information (“PII”), information that could be used to identify a person, with any nonaffiliated third parties other than for the primary purpose for which the information was collected, except with your express permission, or in the limited circumstances described below:

We may disclose information that we believe in good faith is appropriate to cooperate in investigations of fraud or other illegal activity, or to conduct investigations of violations of the Website’s Terms of Use. This means that if we conduct a fraud investigation and conclude that someone who has submitted information to us has engaged in deceptive practices, we may give that person’s contact information to interested parties who request it.

We may disclose information in response to a subpoena, warrant, court order, levy, attachment, order of a court-appointed receiver or other comparable legal process, including subpoenas from private parties in a civil action.

We may disclose information to your agent or legal representative (such as the holder of a power of attorney that you grant, or a guardian appointed for you).
